Overview

The Medical Records Department of the Jewish General Hospital is responsible for keeping your Medical Information. We are one of the first hospitals in Quebec to implement “ Chartmaxx ”, which is a system used to put the paper version of your file onto the computer. We have been scanning documents since 2008.

Services

To obtain a copy of your medical file or that of your child (under the age 14) please complete the entire form Authorization to Release Information and mail it to:

Please be sure to complete the entire form correctly with the patient’s information as to avoid any processing delays concerning the request. Please see the example provided to help you complete the form. If you know your hospital card number (it starts with U-), please indicate it in the space provided on the form.

If you are the parent of a child 14 years and younger, you can sign the authorization form. Please take note that if your child is 14 years or older, your child must give their consent by signing the form.

In specific circumstances, we will need certain legal documents ( birth and/or death certificate, mandate, will, power of attorney, etc) in order to process the request; if you have any questions do not hesitate to contact us.

As we receive a high volume of requests per day, there may be a delay, but rest assured, we will treat it within a reasonable amount of time.

If you come in person, please make sure that you have a hospital card and your Medicare card (or other photo ID) with you.

Once a request for release of medical records has been received,
it could take up to 20 working days to process.


Fees:

  • The first twenty (20) pages of your request are free, we will charge $0.38 per page afterwards.
  • Attestation (Birth, hospitalization, death): $40.00
  • Completion of Insurance Forms: $40.00
  • Date and Time of Birth: $60.00
